EATON COUNTY MEDICAL CARE FACILITY

JOB DESCRIPTION

TITLE:               Director of Social Services                                   STATUS:               Exempt

DEPARTMENT:  Social Services                                                   REPORTS TO:     Administrator

Job Summary:

Under the supervision of the Administrator, the Social Services Director is responsible for the psychosocial wellbeing of the residents and the coordination of admissions. To identify, address and provide for the residents medically related social, emotional and economic problems through personal counseling or by making referrals to appropriate agencies and acting as liaison between the facility and agencies. To uphold and ensure that resident rights, human dignity and personal privacy are enhanced. To prepare and assist resident with plans for discharge and to complete documentation required by Federal, State and facility regulations, and collaborate with the families in order to meet the needs of each resident.

Principal duties and responsibilities:

Performs the following duties listed and others as required to assure quality of life for our residents. This list does not include all duties required but to serve as a basis of expectations. Duties and responsibilities may be added or modified at any time.

Responsible to be a member of the interdisciplinary resident care plan team. Completes resident history, admission assessment, MDS, RAP modules and updates documentation to meet facility and regulatory requirements.

Design a social service program which meets resident needs as well as Federal, State and facility guidelines.

Responds to admission inquiries including facility tours and maintains the waiting list. Coordinates admissions. Gives orientations to new residents and families of the facility relative to facility policies and procedures.

Develops the input from the interdisciplinary team, a discharge plan for each resident according to Federal, State and facility guidelines.

Documents social services progress notes in the medical chart in accordance with regulatory guidelines.

Actively participates in the interdisciplinary care plan conferences by identifying psychosocial issues on the resident care plan and update changes timely.

Interpret, promote, advocate, and protect resident rights, privacy, dignity, and freedom of choice.

Promote and coordinate supportive family networks through monthly/quarterly family council meetings.

Coordinates Inservices and education programs for the staff-related to psychosocial concerns of the elderly and resident rights.

Ensure individual and group counseling to residents and families at times of adjustment, crisis or need.

Establish and maintain resource file which reflects community agencies utilized for referrals. Maintain current data on governmental rules and regulations.

Coordinate resident/family grievance procedures and refer concerns to appropriate Department Head (s) for follow up. Maintain complete grievance report forms.

Promote facility programs and functions in the community. Establish positive interaction within the community.

Supervises Diversional Therapy.

Maintain departmental budget and cost controls.

Reports to the Administrator on resident, family and staff issues.

Performs other duties as required.

Working Conditions:

Long-term care/nursing home setting.

Lifting up to 25 lbs. when needed.

Prolonged standing or walking, Bends, lifts, carries, pushes and pulls various equipment and residents throughout the work day. Extended, unusual or mandated working hours when required.

Qualifications:

Must have a genuine interest in maintaining the facility to assure good quality of life for our residents. Ability to give instructions, organize time and complete tasks with minimal supervision. Ability to manage multiple request and projects simultaneously. Excellent written, verbal and interpersonal skills necessary when interacting with families, friends, physicians and other clinical staff. Knowledgeable of psychosocial needs and concerns of the elderly and an appreciation and interest in working with older adults.

Experience: 2 years experience in long-term care social work required.

Education: MSW Degree and active state licensure required.

 

 

 

Physical Requirements:

Must be able to lift up to 25 lb. Times of prolonged standing or walking, Bends, lifts, carries, pushes and pulls various equipment and residents throughout the work day. This position requires the ability to perform the essential functions described. These functions are not limited to the following requirements.
